Your treatment plan will include a breakdown of procedures to be completed, your estimated co-pay and estimated insurance payment. We offer a variety of payment options to meet your needs. We accept cash and check, as well as the 4 major credit cards, MasterCard, Visa, Discover and American Express.
If you have any questions about financing through CareCredit or payment, ask us! We will thoroughly explain your choices and work hard to accommodate you.
Insurance
Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage (based on the information provided to us) and cannot guarantee payment by your insurance plan due to the complexities of insurance contracts. As a courtesy we will be glad to file your claim on your behalf, all estimated co-pays are due at the time services are rendered. Please keep in mind that your insurance coverage is a contract between you, your employer, and your insurance company, you will be responsible for any amount not covered by your plan.
For your first visit at our office, please bring your current insurance card with you. If you are a current patient and change insurance companies or join another employer’s plan, please be sure to let us know. If an insurance referral is necessary, please bring the completed form with you to your appointment.
We accept most all insurance plans and in most cases will file your claim on your behalf. However, please check with your insurance company to verify if we are in-network with your specific plan.
